Did you just get your passports back recently or was that a while ago?
We didn't have to fill out another form - the clerk just checked the "other" block on the application, with reason "unable to locate".

And I think we got them back the first week of April, or somewhere around there, and we applied mid-January.

    Passport Update!

I applied on June 20 and received mine today, August 17. That makes it 8 weeks, 2 days from dropping it off in Bangor to receiving it back in the mail.

So much for it taking over 12 weeks!

Now I'm a happy International Camper! Woo hoo!

I can't remember how much faster it was supposed to be, but 3 weeks after I called and paid the extra, I had passport in hand.
